Prodware Solutions LLC is seeking a travel nurse RN Case Management Level I for a travel nursing job in Los Angeles, California. Job Description & Requirements - Specialty: Case Management - Discipline: RN - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Position: Registered Nurse – Case Manager (RN) Contract Length: 13 Weeks Schedule: 40 hours/week (5×8 hr shifts) Weekends: Every other weekend Location: Off-site facility (10 miles from main hospital) Weekly Pay Package: Regular Pay$1,920.00 Stipend $1,700.00 Total Weekly Compensation $3,620.00 regular pay for strikes it is $5100 Job Summary: We are seeking a highly skilled and experienced Case Manager RN to oversee patient assessments, discharge planning, and care coordination. This role requires working independently at an off-site location while maintaining strong collaboration with the main hospital’s interdisciplinary team. Key Responsibilities: - Conduct comprehensive patient assessments to determine care needs. - Develop and implement individualized care and discharge plans. - Coordinate care with providers, interdisciplinary teams, and external agencies. - Perform utilization reviews to ensure appropriate levels of care. - Accurately document all case management activities in the EMR. - Advocate for patients to ensure safe and effective care transitions. - Work every other weekend as per the schedule. Requirements: - Active and unrestricted RN license. - Minimum 2 years of Case Management experience (hospital experience preferred). - Strong knowledge of care coordination, discharge planning, and utilization management. - Ability to work independently at an off-site facility. - EMR proficiency.
