Job Title: Community Providers & Services Coordinator

$20.00 - $24.00 SUMMARY: The Community Providers & Services
Coordinator oversees all external service partnerships and referral
systems that support the health and stabilization of Mission guests
and alumni. This position ensures professional coordination of
medical, dental, mental health, wound care, housing referrals, and
government benefits support. This role protects compliance,
strengthens provider partnerships, and ensures guests receive
wraparound care with excellence and accountability. ESSENTIAL
DUTIES AND RESPONSIBILITIES: Medical & Clinical Partnerships
Coordinate onsite medical, dental, mental health, and wound care
services. Maintain partnership agreements with providers. Ensure
proper scheduling and space allocation for service days. Track
utilization and outcomes. Housing & Stabilization Oversee housing
referral processes in and out of Spa 4 area for students and
community members. Coordinate with internal case managers. Track
referral success rates. Maintain partnerships with housing agencies
and placement organizations. Benefits & Government Assistance
Oversee Medicare/Medicaid enrollment support. Ensure compliance
with documentation requirements. Provide oversight for DMV voucher
distribution coordination when tied to stabilization. Alumni
Committee Support Provide administrative and structural support to
Alumni Committee. Assist with long-term reintegration planning.
External Representation & Industry Engagement Attend offsite
meetings, coalitions, and collaborative forums hosted by
healthcare. providers, housing agencies, city officials, and
community partners. Stay informed on industry standards, regulatory
changes, funding opportunities, and new service models. Identify
emerging opportunities that benefit Mission students and the
unhoused community. Represent the Mission professionally in public
and interagency settings. Compliance & Reporting Ensure
HIPAA-aligned confidentiality practices. Maintain service
documentation standards. Provide monthly impact reports to Senior
Director. Monitor KPIs such as: Number of clinical visits Housing
referrals completed Benefit enrollments processed Provider
retention rate Leadership Oversee provider coordination of staff
Develop workflow systems Ensure Christian values are demonstrated
in all service interactions
